Abstract

Background: The increasing consumption of reproductive health content on social media by Generation Z women creates a complex information ecosystem, where the accuracy of content is not always guaranteed due to the absence of scientific verification mechanisms. This condition has the potential to form suboptimal contraceptive literacy and affect the intention of using contraception, while the unmet need for contraception in women aged 15-24 years still persists at 14%. Objective: This study aims to analyze the relationship between exposure to reproductive health content on social media and contraceptive literacy on contraceptive use intention in Generation Z women of childbearing age in Indonesia. Methods: This correlative quantitative study with a cross-sectional design involved 120 Generation Z women aged 18–27 years in the Greater Jakarta area who were selected through purposive sampling. Data were collected online using three validated instruments: the Contraceptive Knowledge Scale (20 items), the Family Planning Attitude Scale (15 items of the Likert scale), and the eHealth Literacy Scale (8 items). The Kolmogorov-Smirnov normality test confirmed the abnormal distribution of the data, so the bivariate analysis used the Spearman Rank correlation test. Results: All respondents showed moderate to high social media exposure. As many as 65% of respondents have good contraceptive literacy, but 35% are still classified as suboptimal. The intention to use contraceptives is mostly in the medium category (80.8%). The Spearman test identified a significant positive relationship between content exposure and use intention (r=0.317; p<0.001), as well as a strong positive relationship between contraceptive literacy and use intention (r=0.593; p<0.001). The advantage of this study compared to the previous study is that it integrates three variables at once in the specific context of Generation Z Indonesia with the eHealth Literacy approach. Conclusions and Suggestions: Contraceptive literacy is proven to be the dominant predictor of the intention of using family planning, beyond just exposure to digital content. It requires collaboration between health professionals, policymakers, and digital platforms to develop evidence-based reproductive health content that is responsive to the information consumption characteristics of Generation Z.
