Dysmenorrhea is the most common gynecological problem among adolescent girls, characterized by menstrual pain that may disrupt daily activities, academic performance, and overall quality of life. The pain is often accompanied by anxiety, stress, and maladaptive coping behaviors. Non-pharmacological approaches such as mindfulness have been proven effective in improving emotional regulation, reducing anxiety, and enhancing coping behavior. However, research focusing on adolescent girls with dysmenorrhea remains limited. To examine the effect of a mindfulness approach on coping behavior and anxiety reduction among adolescent girls experiencing dysmenorrhea. This study employed a quasi-experimental design with a pretest–posttest control group. The sample consisted of 92 respondents (46 in the intervention group and 46 in the control group), aged 15–19 years, diagnosed with primary dysmenorrhea. Research instruments included a coping behavior questionnaire and the Hamilton Anxiety Rating Scale (HARS). Data were analyzed using the Wilcoxon Signed Rank Test and Mann-Whitney U Test. Most respondents were 17 years old, had menarche at 13–14 years, with regular menstrual cycles and menstrual duration of 6–7 days. A significant difference was observed in anxiety scores before and after the mindfulness intervention (p<0.001), with greater reduction in the intervention group compared to the control group. Coping behavior toward dysmenorrhea also improved significantly in the intervention group (p<0.001), while no significant change was found in the control group (p>0.05). The mindfulness approach is effective in reducing anxiety levels and improving coping behavior among adolescent girls with dysmenorrhea. This intervention can be recommended as an easily applicable non-pharmacological strategy in schools and communities to support adolescent reproductive health.
